2.  Exact Model number of motherboard (with PCB revision number and any suffix, if applicable)

3.  Chip type (DIP or PLCC type, and number of pins - Most are 32 pin)

4.  Chip numbers from the top of the BIOS chip, under the BIOS label (29xxxxx, 39xxxxx, 49xxxxx, and other numbers as necessary . . .)

5.  Ship to address, your e-mail address, and payment.
